Form templates let you build your own forms, such as accident reports, detention reports, or inspections, and have drivers fill them out in the app.
Create a template
Go to Forms and open the Templates tab.
Click Add Template. A new template opens as Draft.
Click the pencil icon next to Untitled form to give it a name.
Choose who can use the form
Under Access, choose who can find and fill out the form on their own:
All Drivers – every driver can open it.
Specific Drivers – click Select Drivers and pick the drivers.
Specific Tags – drivers with the tags you choose.
No Drivers – drivers can't open it on their own. Use this for forms you only send with Assign drivers.
Turn on Require approvals if someone must approve or deny each submission before it's final.
Add fields
Every form automatically collects the User and Submission date & time. These can't be edited or removed.
Click a field type in Add to Form on the left, or click + Add field. Available fields:
Instructions – formatted text to guide the driver.
Text, Long text, Number, Phone number, Date
Single select and Multiple choice
Acknowledgement checkbox – for the driver to confirm they've read something.
Media – photos or files.
Signature
Driver, Vehicle, Trailer – pick from your records.
Turn on Required for fields that must be filled in.
Drag the dotted handle on the left to reorder fields.
Click the red trash icon to remove a field.
Use + Add section to group fields, or + Add table for rows of repeated information.
Save and publish
Click Save.
When the form is ready for drivers, click Publish in the top-right corner. Published templates show as Published in the Templates tab, and the button changes to Unpublish.
Manage templates
In the Templates tab, each template shows its Status and Access. Use the icons in the Action column to edit (pencil), assign drivers (person icon), or copy the template.
Note: Drivers see forms in the app only when Forms is turned on in Settings > Driver App.
